This policy explains what personal information I collect through drhaleema.com, why, where it is kept and the choices you have. "I", "me" and "my" mean Dr. Haleema Khan, chiropractor, Dubai, United Arab Emirates. Questions about your information can be sent to drhaleema@drhaleema.com.
What I collect
When you use a booking or offer form on this website, I collect:
- your name, mobile number and email address
- your preferred appointment time
- your consent to me keeping these details
- for the offer page, which advert you arrived from (for example "Instagram"), so I can tell which adverts are useful
The forms do not ask about your health. Your history is taken in person at your first visit, so please do not send medical details through the website.
If you message me on WhatsApp or by email, I also receive what you send, and WhatsApp and your email provider handle those messages under their own privacy policies.
Why I use it
- to contact you, arrange your appointment and answer your questions
- to follow up on your care, if you become a patient
- to understand which adverts bring people to the website
I rely on your consent, which you give by ticking the box on the form. I do not sell your information, and I do not use it for marketing unless you ask me to.
Where it is kept
Form submissions are stored by Netlify, the company that hosts this website, and a copy is emailed to me through Web3Forms, an email delivery service. Both may use servers outside the UAE. Only I can access the submissions. Clinical records created at your appointments are kept separately, in line with Dubai Health Authority requirements for patient records.
How long I keep it
If your enquiry does not lead to an appointment, I delete it within 12 months. If you become a patient, your information becomes part of your clinical record and is kept for as long as the law requires.
Cookies and other services
This website does not set advertising or tracking cookies. Its fonts, images and code are all served from this website itself.
Your rights
You can ask me to show you the information I hold about you, correct it, delete it, or stop using it, and you can withdraw your consent at any time. Email drhaleema@drhaleema.com and I will respond within 30 days. Withdrawing consent does not affect anything done before you withdrew it.
Changes
If this policy changes, the updated version will appear on this page with a new date.
